Are you a provider of services for Iowans aged 60 and over or their caregivers? Would you like to be included in the Search for Services database on this web site? Are you already listed in our Search for Services database but would like to update that information?

The forms and documents found here will assist you in being part of this valuable referral service for older Iowans and their loved ones.

Steps:
  1. Read the Inclusion/Exclusion Criteria to make sure your service qualifies to be listed.
  2. Review the Types of Services Included in Search for Services Database
  3. Complete the Intake Form entitled: Overall Agency Information
  4. Complete the applicable Services Intake Forms
  5. Please repeat this process annually (or sooner, if your information changes)

How long will this take?

10 – 30 minutes of your time to complete the necessary forms, depending on the variety of services you offer.
